Prognostic Value of Day 3 Estradiol on in Vitro Fertilization Outcome

Abstract
This study was performed to evaluate the prognostic value of day3 Estradiol (E2) levels, independent of day3 FSH levels, on response to ovulation induction and subsequent pregancy rates (PRs) in IVF-ET patients.
From Jan 1994 to Dec 1995, 90 patients who underwent 114 IVF cycles with luteal phase GnRH agonist suppersion and HMG stimulation were allocated to the study group. We evaluated response and outcome data including age, day3 FSH and E2 levels from
a
menstrual cycle before IVF, ampules of HMC used, maximum E2 level, cancellation rates, and clinical pregnancy.
Despite similar age, number of ampules of HMG, and peak E2 levels, patients with an elevated E2 level (E2¡Ã50pg/ml) (conversion factor to SI unit 3.67) and had a higer cancellation rate (10.0% versus 1.0%) compared with those with E2 level
¡Â50pg/ml.
Even with patients with elevated FSH levels (FSH¡Ã15mIU/ml) (conversion facto to SI Unit, 1.00) were excluded (leaving 105 cycle), those with an elevated day3 E2 still had a lower PR per initiated cycle (7.4% versus 1.0%).
Therefore this result suggested that patients who presented with an elevated day3 E2 (¡Ã50pg/ml) in a cycle before IVF-ET had a higer cancellation rate and a lower PR indepedent of FSH. A day3 E2 level, in addition to a day3 FSH level, appears
very
helpful in prediction in IVF-ET outcome.
